DIAKON COMMUNITY SERVICES FOR SENIORS APPRISE PROGRAM IS HERE TO HELP. 

Do you or someone you love have or need a Medicare Prescription Drug Plan (Medicare Part “D”)?  If so, then this announcement is for you!

Each year the prescription drug plans may make changes to their formularies, monthly premiums, deductibles, etc.  Your chance to enroll, disenroll or change plans is during the annual election period, which is between Nov. 15th and Dec. 31st each year.

In order to assist you in reviewing your options, the Schuylkill County APPRISE program will be holding comparison appointments during the open enrollment period, to assist you in making these important decisions. The APPRISE Program offers free, confidential health insurance counseling for persons with Medicare and is managed by Diakon Lutheran Social Ministries under a contract with the Schuylkill County Office of Senior Services.

These appointments will allow you to compare your options among all the plans that are available.  In order to provide the most accurate comparisons, you will need to bring your Medicare Card, any other health insurance cards you may have and a list of your current prescription medications, including the dosage and the quantity in a 30-day supply to the event.
